Please read it carefully before you start treatment and bring a signed copy with you to your first session. Again, another important document that will make your therapy in my office more effective. This document allows you to describe information about you that I should know before we start the therapy. Please fill out as much as possible and bring it with you to your first session. If you have any questions regarding this document feel free to ask them during our first session.
Both these documents require that your computer has Adobe Reader installed. If you can open the documents by clicking on the links above and see their content, then you have Adobe Reader installed. If this doesn't happen, you can just download the free Reader from Adobe's website.
Insurances Accepted
I am credentialed with the following insurance companies as a network provider. Sometimes this is also referred to as "being on an insurance panel".
If you are insured with one of the following companies please contact them to find out about their reimbursement options for "network providers" like me.

If I am not on your insurance panel, I can provide you with a billing statement in order to request reimbursement from your insurance company. Please note that I do have a 24 hour cancellation policy.
